Additionally, you will need to submit a valid driver’s license number and your social security number.  If there are additional partners or officers associated with your business, you will need to provide this information for them as well. You will be asked to state whether you are applying for a new business, a change of ownership, or a change of address. You need to declare the type of business meaning whether you are a sole proprietor, in a partnership or part of a larger corporation. The application asks for two reputation and character references and their contact information. If your business location was previously owned or occupied as another a business, you will need to state what the previous business was and supply the business property owner’s name. 

It is very important that all information is true and accurate. If you knowingly provide false information on your application, you could risk being denied your business tax certificate or having it revoked.  By signing the application, you are stating that all information is true and accurate to the best of your knowledge, including your estimated gross receipts. After completing your business tax certificate application, you will need to complete a commercial/industrial use questionnaire. 

This application can be filled in and printed from the city website. It is a detailed questionnaire that asks questions such as what your proposed business activities will include, your name and contact information, the name of your property owner and his or her contact information. There are questions concerning the number of employees you plan to hire, what types of tools and equipment you will use, your hours of operation, alarm system information, vehicle information, etc. Leaving any questions blank on this application will automatically result in your application being denied. Once you have completed your applications for your business tax certificate, you will need to return it to the City of Hawthorne, 4455 West 126th Street, where it will be reviewed and then handed to the Planning Department. If it is approved, it will be sent to the Building and Safety Department where an on-site inspection will be scheduled to be sure that your building complies with city safety regulations. Providing your property passes this inspection, your application will then be passed to the Fire Department for fire inspections to be sure that you are in compliance with fire codes. 

What costs can you expect to incur?

The fees associated with your business tax certificate will be calculated based on your annual gross receipts.  You caan find how much you wil need to pay based on your gross receipts by looking at the City of Hawthorne website under Gross Receipts Schedule. There may be other fees for inspections and permits, which will need to be payed when you return your application.
